Anemia is a laboratory diagnosis that is poorly related to symptomology. Patients with gradual onset may be asymptomatic with quite low Hb/HCT, while acute onset can cause symptoms with relatively less anemia. Higher Hb/HCT is seen in patients living at high altitudes, smokers, and patients living in air pollution areas. Endurance athletes have increased HCT. Lower Hb/HCT is seen in African-Americans, elderly, and pregnant women. Anemia can be associated with hypothyroidism, depression, cardiac causes, and pulmonary causes of SOB/DOE. Chronic fatigue syndrome and others are also considered. Differential diagnosis includes pallor, tachycardia, bounding pulses, systolic flow murmur, and glossitis. Physical examination includes CBC, reticulocyte count, and peripheral blood smear.
